Private help while in a facility
Whether you or a loved one are residing in a Retirement Home, Long Term Care facility, or even Hospital, our talented and dedicated team are committed to providing compassionate care to ensure that you get the best possible supports for your individual needs.
Retirement residence supports: We can provide personalized services to ensure that you have the best possible experience in retirement living. Services include Emotional and mental support including one-on-one companionship, crafts, games, or even promoting and attending group activities or events. Maybe even a drive in the country or to the local supermarket to get a change of scenery.
Long Term Care support: Everyone deserves to be treated with respect and dignity, and sometimes a little extra care with a personal touch can go a long way. Our care management team can create a plan built around your loved one’s own background, preferences, and needs. Services include companion care, personal care, and family respite. We provide peace of mind that someone is there for your loved one when you aren’t available.
Hospital support: We recognize the unique challenges faced while in hospital. Our team is dedicated to ensuring that you are not alone during this difficult time. We understand that hospital staff are busy, and days can be long and lonely. Our care team is there to help! Services include companion care and emotional support, assistance with toileting, personal care, physiotherapy routines and range of motion, and so much more.
Motor Vehicle Accident (MVA) Care
Accidents happen! And when they do, the impact can be life changing for you and your loved ones. The Memories+ team understands that there are specific challenges that come along with post accident recovery. Our healthcare team is here to help you every step of the way.
Our nurse case managers will create a customized care plan and work with you to support all your healthcare needs in your journey to recovery. And if you are covered by insurance, we will work with your insurance provider to ensure that your needs are met based on your policy’s coverage. We can often even invoice your insurance provider directly!
Navigation of Publicly funded care services
Many of our clients receive publicly funded care through Home and Community Care Support Services (HCCSS) formerly known as the LHIN, and if they don’t, they may be eligible for some level of care available through provincial funding. We work very closely with HCCSS coordinators to ensure that our clients receive the care that they need and may be eligible for, and that our private care services work in conjunction with other providers. If you or a loved one are unsure about what is available to you, one of our talented case managers would be happy to help.
Depending on your circumstances and/or location, you may even be eligible for the Family Managed Care (FMC) program. This program offers you greater flexibility to choose your own care provider, determine how the care plan is delivered and handle all related administrative tasks on your own. We can help you through this process or direct you to a specialist who can do it for you.
Staffing relief services
We realize that there are challenges with staffing in all aspects of healthcare, and as needs increase, these challenges become even greater. The Memories+ care team is here to help. We have a staff of certified PSWs and Nurses able to support your needs in Retirement Homes and Long-Term Care facilities.
Our staff go through stringent screening and onboarding process to ensure that we only offer the most qualified caregivers for any required role.

Specialized Nursing Care
Whether you are managing a chronic disease or recovering from an illness, surgery, or a serious injury, our nurse managed care team is here to help. Most of our services can be provided by a Personal Support Worker (PSW), however if your needs or more complex, or your health insurance policy specifies registered care, we have a team of nurses ready to help.
Our nurses are not only experienced in many aspects of their trade, but also go though stringent screening to ensure that you receive the best possible care for your situation.
Services include Catheter and ostomy care, wound care and dressings, diabetic care management, Medications management, Vital signs monitoring, and complex care management.
Naturally Occurring Retirement Communities (NORCs)
Naturally Occurring Retirement Communities (NORCs) are a growing phenomenon in Canada, reflecting a trend towards aging in place. Unlike traditional retirement communities that are built specifically for seniors, NORCs evolve organically as a significant number of residents within a community age together – often in urban apartment buildings or pockets of suburban neighborhoods. The development of NORCs is particularly noted in urban settings where high-rise apartment complexes see an increase in the older adult population.
While NORCs offer many advantages, allowing folks to stay close to their social network, they also present unique challenges for providing care services. With proactive planning, accessibility to healthcare and emergency services can be tailored to mor effectively support Naturally Occurring Retirement Communities. When a service provider is located within a building housing a large population of older adults, often a unique model of care can be implemented allowing a more efficient and cost-effective supply of services to those in need. As an example, nurse managed care can be supplied in much shorter blocks of time, eliminating the need for multi-hour minimums.
The Memories+ Care Management team is well aware of the growing need in these communities and is poised to support this need with a modified care model tailored to each individual community’s needs.
