Nurse Case Management – Our Gerontological nurse case manager will get to know you, your physical, mental, and emotional needs, and everything you desire as part of your care services. This specialized level of training and insight gives our client’s families peace of mind that their loved ones’ care is always relevant to their needs and is provided as a part of regular services.
Nursing Foot Care – Healthy feet allow you to be active and have numerous benefits such as keeping weight off, maintaining and improving muscle and bone strength and improving your emotional and mental health. Our Foot Care services are provided by nurses certified in advanced foot care training and experienced in conditions such as diabetes which can be the cause of many sensitive issues related to the feet.
Respite Care – We understand that people are more comfortable and tend to do better in their home environment. If your loved one has chosen to stay in the comfort of their own home but needs assistance to do so, Memories+ gives you the freedom to choose from many different options. One of our knowledgeable case managers would be happy to speak with you and help you to determine what option is best for you.
Nursing Care Services – Our nurse will take the time to discuss your health issues with you and explain any procedures in a patient and respectful way. Hospital Discharge – We are always happy to have our nurse attend a discharge meeting before leaving the hospital. Attending this meeting allows us to understand our client’s needs best and ensure a smooth transition home by coordinating duties such as transportation home, ordering and delivering medications, setting up any required home modifications, and providing in-home care services are in place upon your return.
Gerontological Nurse – A Gerontological nurse focuses on the care of seniors and is educated to understand the varied health needs of older adults, which can change and evolve and present themselves quite differently than with other age groups. Gerontological Nurses provide comprehensive assessments and treat the complex physical and mental health needs of older adults. They promote overall health and wellbeing and address determinants of health such as the social and economic environment, the physical environment and the person’s characteristics and behavior’s. The Gerontological Nurse works directly with older persons and their families, linking them to needed services.
Adult Day Program – Memories Plus offers mental stimulation and meaningful engagement through music therapy, art therapy, pet therapy, laughter therapy, fitness programs, recreational therapy, and social meetings.
Personal Care – Our caregivers know how to handle these situations respectfully, helping you maintain dignity throughout your day while promoting independence in all that we do.
Recreational Therapy – Recreation Therapy is dedicated to helping people overcome health barriers and engage in healthy and rewarding recreation programs. A Certified Therapeutic Recreation Specialist facilitates our in-home Recreation Therapy services. Through the administration of a Recreation Therapy Assessment, a plan is put in place to implement and promote purposeful, meaningful, and engaging one-on-one recreation programs.
Live-in Care – We understand that people are more comfortable and tend to do better in their own environment. If your loved one has chosen to stay in the comfort of their own home but needs assistance to do so, Memories+ gives you the freedom to choose from many different options. Live-in care can be a cost-effective alternative to hourly care if help may be required around the clock.
Positive Approach to Care – Memories+ Group is committed to changing the culture of dementia care that we offer.
